New PopHealth Management Analytics for Clinicians

Apervita, Inc. announced that the DARTNet Institute has joined the growing list of health leaders to leverage Apervita to create and distribute affordable evidence-based analytics to primary care practices for population health management.

DARTNet is a not-for-profit research support organization, collaborating with one of the largest groups of practice-based research networks. The DARTNet collaborative has built a national data collection network for electronic health records, claims and patient-reported outcomes to assist with research, quality improvement and clinical care. As part of its partnership with Apervita, the DARTNet Chronic Disease Screening & Management Protocols have been made executable so they can be used by primary care physician (PCP) practices across the country.

“This partnership with Apervita allows physicians to access and deploy DARTNet’s analytics in a way that is relevant to managing the early onset of chronic disease,” explained Guillermo Rocha, MD, one of DARTNet’s participating physicians. “Many PCPs feel overwhelmed with the amount of patient data they have to deal with today. Protocols that process complex data have helped identify risks and support us in adhering to best practices.”

Chronic Kidney Disease (CKD) and End-Stage Renal Disease are increasing in prevalence across the United States, and contribute to growing healthcare costs, according to the National Kidney Foundation (NKF). Evidence from the NKF Kidney Disease Outcomes Quality Initiative (KDOQI) suggests that the early detection and management of CKD and its associated co-morbidities, such as hypertension, dyslipidemia, cardiovascular disease and bone disease, can improve patient outcomes by delaying disease progression and improving quality of life. With evidence-based protocols, DARTNet provides PCPs with tools that facilitate adherence to the recent guidelines set forth by the NKF KDOQI.
